HORSE SHOE KIDNEY - DETECTED AT 38 YEARS

A 38 years old male with history suggestive of left ureteric colic with typical pain left lumbar region with radiation to left testis and medial side of upper thigh . USG - Horseshoe kkdney with fused lower poles and a left lower 6 mm ureteric calculus with mild hydronephrosis. The case is being posted that this man 38 years old and at this age this congenital anomaly - horse shoe kidney was detected as now he became symptomatic due to a lower ureteric calculus

Horse shoe shaped kidney is the development of kidneys due fusion of kidneys at tha lower pole as isthmus. This malformation of kidneys lead to malrotation of kineyd, high insertion of ureter and when crosses the isthmus there is obstruction of the collecting system of urine causing stasis of urine leading to increased obstruction of uretero pelvic junction and urinary tract infection casing formation of ureteric stone. So pt was asymptomatic as there was no stone ureter and no obstruction was there but horse shoe shaped kidneys are prone to develop stone due anatomical abnomality if the course of ureter.and there is stone formation there is pain

A rare case Sir. Supportive treatment. B-coz uretric calculas of 6 mm with Hydronephrosis. I think Surgery is the best option.

Anybody can develop Ureteric calculi.A lower ureteric calculus of 6mm has a 80%,+ chance of spontaneous passages .All he needs is MET. The commonest problem in horseshoe kidney is a puj obstruction. The fact that the stone come down to lower ureter is that there is no PUJ obstruction. All inall do nothing for the Horseshoe part Only warn him to beware about injuries to abdomen

Tab. Systone 1 tab tds continue. Tab. Liv52 1 tab BDAC continue. Antispasmodic SOS. Water intake adequate. Change the life style.

Needs Conservative treatment. Avoid abdominal injury and extra stretching . Few cases seen Thanks for updating.
