A female with acute onset GTCS
A female aged 45 years Chief Complaints One episode of GTCS 2 hrs back Headache (x2 days) Physical Examination GCS - E4V4M6 No FND or hemiparesis Investigations MRI Brain
Late onset single episode of fit. MRI brain - Full pictures not posted. ONLY T1W AND FLAIR images available.T2w image and contrast MRI is needed for full discussion. T1w images shows isointense with peripheral hypotension lesion in the left parietal lobe. FLAIR image shows well defind predominantly hyperintense lesion.Also left frontal subgalial swelling. Needs contrast MRI .
Wish there were more images in more planes,My DDx Meningioma,secondaly low grade glioma.Lesion likely grown over many years,finally resulting in seizure activity.
Lt cerebral hemisphere shows a gliotic area,which could be reason for seizure.inj valprol 1000 mg iv stat f/b inj valprol 500 mg iv bd,inj mannitol 100 ml iv tds,tab diamox 400 mg BD

New case 42 yr ,M, presented with one episode of left sided focal fit starting from left lower limb on 26 th of last month.Focal fit lasted for 10 mts and he developed left sided weakness soon after the fit. SINCE THEN HAVING ALMOST CONTINUOUS IRRITATING TYPE OF HEADCACHE ON THE RT SIDE OF HEAD. No vomiting. No previous history of seizures or any medical illness. On exam vitals stable. Normal higher functions and speech . Normal optic fundi. Mils obliteration of left nasolabial fold. Motor system gr 3/ 5 left side with DTRs +++,with rxyensor plantar lt side . Intact sensations. What abnormality in theMRzi brain?

New case. 47 yr old ,F, presented with bifrontal head ache since 7 months Most of the time she has head ache but waxing and weaning in intensity with out any nausia / vomiting / vertigo. She took Amitriptyline,propranolol and paracetamol with out any relief. No history of any significant medical illness. On exam vitals stable BP 130/ 80 mmhg. Optic fundi normal . Nonlong tract signs. No neck stiffness. DONE MRO BRAIN WITH CONTRAST. Diagnosis and management.
