BASAL CELL CARCINOMA?

A non - healing ulcerative infected lesion on right side of nose just below medial canhus of right eye Chief Complaints A 68 years old female - a tribal nomad , with a non - painful lesion over right side of nose since 5 months History There was a very small nodular lesion over right aspect nose below medial canthus of right eye which after scratching became a non - healing ulcer and gradually increased to present size . Vitals WNL Physical Examination An ulcer- proliferative lesion over external surface of right nose just below medial canthus of right eye with crusts and sero - sanguinous discharge . Mild tenderness- most likely due to secondary infection Investigations Not done Diagnosis Basal cell carcinoma? Management As patient is reluctant to get biopsy done - and not co-operating for any invasive procedure- management remains difficult

You are absolutely correct most likely this is BCC As pt is not co operative we have to play our tricks to get punch biopsy or scrap biopsy for their benefits so as l8ne of treatment can be designed Most like we need chemoradiotherapy

Basal cell Carcinoma. Needs further investigation and evaluation to conclude. Biopsy under consent of his attendants. Chemoradiotherapy required.

Only biopsy can detect that it is Rodent ulcer or Basal cell carcinoma

Picture suggests clinically cell carcinoma.For confirmation biopsy is a must.Proper counseling is needed for biopsy.Once reported as malignant (B C C) treatment choices would be firstly Surgery under surgical oncologist. Radiation to be followed taking care of eyes with proper planning. (Chemotherapy may also be tried with the opinion of medical oncologist). Results with surgery and radiotherapy will give good results
