A patient aged 34 years came to me with hypertrophic lesions on both legs and on back..which is itchy in nature. lesion first appear on one leg 2years back and now occured on both leg and on back.. i have prescribed these medications.. ointment clop s caladryl lotion cap evion 400 mg O.D tab atarax 25 mg h.s kindly give your suggestions...
Sir, my impression is Hypertrophic Lichen planus 1. If the itching is severe and lesions are increasing a short course of oral steroids are helpful 2. If itching is moderate and lesions are not progressing topical clip S with atarax should help. 3. Caladryl and evion are of no use. 4. Lichen planus usually self resolve in 1 to 3 years. 5. Steroids( topical and oral) are the main treatment.

66y/o female patient presented with complaints of persistent itching and multiple Hyperpigmented Skin lesions both legs (from thigh to feet) lasting over 4 months. The patient has Mhx of HTN, non diabetic, currently on Amlodipine and Lisinopril which she has been using for more than a year. PMHx: Severe itching on both hands and legs with mild swelling 4years ago. She was treated and symptoms subsided. Pt has seen a dermatologist, no diagnosis was given. labs (LFT, E/U/Cr, Pap smear) are non-significant. No mention of a biopsy. Pls, what's your advise?? (Patient tends to believe it's spiritual problem)
