A patient with a history of anorexia nervosa presents, who has been treated successfully with regard to weight gain, including inpatient stay ~5 yrs ago. She has an ongoing generalized anxiety disorder and takes an SSRI. Over the past 6-8 months, however, she has become more pre-occupied with weight and exercise, focusing on gaining muscle, and only eating 'pure' foods. She has been maintaining/gaining weight so I am not concerned about retreat into anorexia. She does not meet criteria for OCD. Orthorexia is not currently recognised in the DSM but wondered what your approach would be to this kind of patient? She has declined CBT.
