Cor triatriatum is a rare developmental anomaly in which a membrane divides the atrium and separates the pulmonary veins from the mitral valve (CTS) or, less commonly, the caval veins from the tricuspid valve (cor triatriatum dexter). CTS is believed to reflect failure of incorporation of the common pulmonary vein into the left atrium. This failure results in a variably obstructive membrane at the junction between these two embryologic structures, with a proximal chamber inclusive of the pulmonary veins and their confluence and a distal chamber reflecting the true left atrium and left atrial appendage.
One of the rare congenital cardiac defects, less commonly known to general practitioners. Patient is lucky- not to have associated other cardiac anomalies Cor triatrium sinistrum is more common as in this case,. as there is typically a proximal chamber that receives the pulmonic veins and a distal (true) chamber located more anteriorly where it empties into the mitral valve. The membrane that separates the atrium into two parts varies significantly in size and shape. It may appear similar to a diaphragm or be funnel-shaped, bandlike, entirely intact (imperforate) or contain one or more openings (fenestrations) ranging from small, restrictive-type to large and widely open.. 51 year old female presents with red rash on bilateral lower extremities. Pt also presented with swollen feet/ankles with +4 putting edema. Pt states the swelling gets worse progressively throughout the day and is the worst at night but improves upon waking. Pt states she is tired most of the time and has shortness of breath upon exertion. Pt has a history of a hypertensive crisis that required a hospitalization.
