A young male with Disseminated Malignancy
A young male aged 34 years Chief Complaints Pain Left Testes Jaundice Pain Abdomen (x 2 months) Physical Examination Bulky Left Testes Pall(++) Investigations PET-CT
Obviously need more evaluation such as microscopic study
Malignancy of testis With metastasis in liver Jaundice Opinion of oncologist
PET CT indicated increase take up of isomer suggestive of seminoma testes with malignant liver
Needs evaluation for primary focus of neoplastic etiology.
This is a rather urgent situation. Likely NSGCT but get markers, AFP, bHCG, LDH. Will need orchidectomy, and definitely chemotherapy (BEP or VIP). This is likely poor risk ( if NSGCT) but curable. Need to move fast

This is an x-ray chest PA view from a 26-year-old boy who presented with a left scrotal swelling for 6 months and low grade fever for 2 months. On examination, built and nourishment is below average, afebrile. No lymphadenopathy, clubbing. Mild pallor present. A swelling of size 2 X 3 cm is found in left scrotum arising from left spermatic cord, non tender with no local rise of temperature. Rest of the systems WNL. The case is open for discussion.
