76 years olf female - a known case of IHD and hypertension on regular treatment on IHD protoco l complains of sudden onset right upper limb paresis and slight deterioration of consciousness level . MRI -- scan - Acute left basal ganglia infarct Encephalomalacia with gliosis in left occipital and Parietal lobe Old lacunar infarcts bilateral periventricular area Opinion welcomed
