“COVID-19 & Digestive System” The major highlights from these data are as below: 1. When COVID-19 epidemic began, initially we thought that GI symptoms would be less prevalent in COVID-19. However, now data are emerging indicating that more than 20% of COVID-19 patients may also be presented with digestive symptoms. Diarrhea, being the most commonly reported GI symptoms in around 20% of COVID-19 patients (excluding lack of appetite which is not considered as a specific GI symptom). 2. Few COVID-19 patients may also be presented with only digestive symptoms without any respiratory symptoms (3% of COVID-19 patients according Am J Gastroenterol paper) 3. Around 10% of laboratory positive COVID-19 patients might not have digestive as well as respiratory symptoms. 4. Patients with digestive symptoms may take longer time from onset of symptom to hospital admission. 5. Patients with digestive symptoms may have elevated liver enzyme compared to patients without digestive symptoms. 6. COVID-19 patients presented with GI symptoms are usually elder, having more comorbid condition, suffering from severe Covid infection, more chance of being on mechanical ventilation, having elevated ESR, and at more risk of death compared patients reported without diarrhea. 7. Meta-analysis of 16 studies, more than 4200 COVID-19 patients: – Virus RNA was detected in stool samples from 48% patients. 70% of patients had persistently positive stool RNA even after respiratory tests had become negative Take home: GI symptoms are common (and can be the presenting manifestation) and that stool viral shedding occurs throughout disease course (and may persist beyond it). Thus, attention to preventing fecal-oral transmission of COVID-19 is crucial. I have incorporated data from following studies/articles here:- 1.
