Caffeine and Aminophylline can be used together in apnea of prematurity???
as i have read u have already ruled out many things which can predispose to apnea . baby might b having intermittent seizures too . i would advise u to do usg cranium too .
Not read about simultaneous administration anywhere in standard textbooks. Caffeine is well tolerated & has less side effects as compared with other aminophylline. Please share , in case you find any text...
You should avoid aminophylline in Neonates. Caffeine in the doses of 5-10 mg/kg/day - either once daily or twice daily (divided) should be enough. In cases of frequent episodes of apnea - rule out sepsis, hypoxia, hypoglycemia, electrolyte imbalance, positional apnea, Gastro-oesophagial reflux etc and treat accordingly. Good luck
The study of Henderson-Smart et al. on 108 premature neonates with prematurity apnea showed that there is no significant difference between the effects of treatment with caffeine and aminophylline
AVAILABLE EVIDENCES SHOWS THAT.. CAFFEINE IS AS EFFECTIVE AS.. IV THEOPHYLLINE..
Caffeine is more effective than aminophylline.. But it is not used due to high price
Caffeine is much more prescribed or used than theophylline unless necessary...

5day old male came to my opd. LSCS delivered for foetal distress in Govt hospital . Cried at birthday but lethargic. Developed subtle seizures on 2nd day. Not controlled by phenobarb load and maintenance at hospital. Referred out. Sepsis screen -ve, RBS and calcium normal. Peculiar wrinkled, dry skin .poor feeding. Shrill cry. Poorly sucking. Poor tone. Mother has a healthy 5yr old daughter. Suggest D/D
