I think let's simplify it,u r having a malnourished child c bacterial diarrhoea,u need to treat it as sepsis.choice of antibiotic will be mikacin,and floroquinolone as I'm suspecting shigella.more over I'm bothered about cause of malnutrition,is it bottle feeding or improper weaning.
Occult blood in stools may be due to go infections like e coli , or hookworm or pinworm infestation, etc Cow milk intake also can be the offending agent. Most of the E Coli diarrhoea s subsides on their own. Symptomatic treatment needed . A repeat test after a week

This 44 year old male (see connected case) was started on Levofloxacin, Doxycycline and Tamiflu (Oseltamivir). However, his fever persisted and dyspnea worsened. His blood pressure dropped to 84/50 mmHg after intubation, and vasopressors were given via CVC (central venous catheter). A 12-lead ECG revealed sinus tachycardia and the CXR obtained 2 days later (attached) revealed diffuse infiltrations over both lungs. HBsAg and anti-HCV were negative. The abdominal echo showed no lesions of the liver, gallbladder or biliary tract. Coronary angiography (performed due to elevated cardiac enzymes) did not show coronary artery thrombosis. Lumbar puncture showed no evidence of central nervous system infection. PCWP was 16 mmHg. A dx. was made of pneumonia, septic shock, ARDS and myocarditis. Two days later, lab results came back negative for dengue fever, Q fever, scrub typhus, Rickettsia, but was positive for influenza B. What is the next step in the management of this pt.?

A 8 yr mch with a h/o fever since last 15 days . 3-4 spikes a day. Except for the itchy rashes shown , no other complaints. Has been given lumerax, amoxyclav, cefixime, ofloxacin earlier by the previous doctor. No localising signs. Child is playful after oral paracetamol. No animal exposure. HT and wt are age appropriate. He has come for consultation today. I have stopped all drugs. Advised SOS paracetamol. Investigations seem normal. How should I proceed further?

patient is 39 F with most recent undetectable VL and CD4>500 last month presenting with diffuse joint pain. She has diffuse rash starting in her lower extremity. She has fever and chills.
