33 y/o female with chest pain, difficulty on breathing, and heart palpitation. diagnosis???

Patient presented to urgent care today with a cough x3 weeks. No other symptoms. No significant past medical history. We referred her to a pulmonologist. Our radiologist said the nodules are consistent with metastatic disease. The largest measuring at 3.9 cm. Thoughts?
