Female, 80 years old. Four days ago she felt a severe pain in the inguinal region and hip on the left side. After a time she noticed the appearance of the lesions illustrated above. The doctor who evaluated the patient started treatment with acyclovir 200 mg every 8 hours. What is the diagnosis? Is the treatment correct?

A case of Herpes Zoster naturally. Treatment is not right. Tablet Acyclovir 800 mg 4 th hrly- 5 doses. Miss the midnight dose. Symptomatic treatment for pain. Amitryptiline 25 mgs Hs to reduce burning sensation . Fusidic acid cream externally. External application of Acyclovir doesn’t have great benefits. Since the age is 80 yrs look for immuno compromised states. Advise to drink plenty of water.

Herpes zoster Acyclovir 200mg 5times a day. Clean with betadine lotion apply calamine lotion+ xylocaine 2%gel locally. Acyclovir oint locally. Probiotic and multivitamins orally. Analgesics if pain. Improve general and personal hygiene. Nutritious balanced diet.

A case of Herpes Zoster no doubt. The lady being old investigate for immuno compromised states. The treatment is under dosed. The treatment recommended shall be tablet Acyclovir 800 mg 4 th hrly 5 doses (avoid the midnight dose). For a period of 7 to 10 days.symptomatically for pain, Amitryptiline 25 mg to avoid the burning sensation. Externally fusidic acid. It’s observed in majority of cases external application of Acyclovir doesn’t help in a big way. Review after a week

Hepes zoster . Treatment is correct. May need further investigations to rule out Possible DM / Malignancy which is common with HZ , particularly in this age group.
