Admitted in ICU First check hemotocrit value if it's increase then patient in DSS Measure colour , capillary refill time ,temp ,valume of pulse ,rate of pulse ,BP ,I/O If bleeding not present manage with Crystalloids (NS /RL ) If bleeding occurred then transfuse platelets (( indications of platletes transfusion ..... Platletes count less than 50000 with bleeding or less than 10000 without bleeding ))

Better treat the patient as indoor admired with plenty of IV fluid...... Repeat check of Platelets.. If no desirable increase in Platelet count manage for Plasma infusion.

Keep a check on hematocrit and serum ferritin, these are valuable indicators of how disease is progressing, and predict multi organ involvement. IV fluids to be given judiciously. Platelet count of 22000 doesn't need any intervention right now, but counsel the attendants for platelet transfusion. LCR (large cell ratio) parameter in the hemogram will predict if you will need platelet transfusion in the future or not. If LCR starts rising or even stops falling, means platelets count will stabilize in the future. Watch for any evidence of bleeding. Absolute indication of platelet transfusion is count less than 8000, or less than 20000 with bleeding. Other than that IV fluids, PCM oral or infusion, is the supportive treatment for dengue. Use of carica papaya and Sat Giloy for increasing platelet count is debatable and is a personal choice.

Sir platelet count has no correlation with severity of disease unless it's too low to cause hemorrhage. Under all circumstances patient needs to be admitted One needs to look for danger signs of Dengue Then check vitals... Signs like tachycardia, low BP, NARROW pulse pressure, and organomegaly needs to be noted PCV and platelet count needs to be checked frequently as 4hrly as the case demands Entire management cannot be written here. There is widely available on internet- dengue protocol state specific and GOI specific and WHO specific... That needs to be followed.
