Lesions over the neck

A 45-year-old obese female presented with such lesions over her neck region with mild itching. She attributes it to wearing a necklace which led to this. So it gave me an impression of allergic contact dermatitis. After a week of treatment, she is not doing fine. What would be the diagnosis? ACD? Nodulocystic truncal acne? Kindly comment on the lesion and line of management? She also has a history of acanthosis nigricans.

Yes, it could have been diognised as contact allergy to cosmatic s..but still persists..likely to be Acanthosis nigricans. A skin condition characterised by dark, velvety patches in body folds and creases. Acanthosis nigricans typically occurs in people who are obese or have diabetes. More rarely, it can be a warning sign of a cancerous tumour in an internal organ, such as the stomach or liver.

Acne incognito Isotretinoin 20 mg twice daily ten days then once a day next 20 days Benzyl peroxide 4%gel 2 hours in the evenings

Acanthosis Nigricans with Nodulocystic Acne vulgaris ( extending upto Trunk). Rule out Diabetes Mellitus.

DX Acanthosis nigricans ..hyper pigmentation ,acne vulgaris extended to trunk and skin tags...
