Lung and renal mass

Chief Complaint A 30 y/o female presents with the complaint of severe dyspnea. History She is diabetic, non-smoker, non-acoholic female. No other past medical history. Had 1 cesarian delivery 3 years back. Child is healthy. Vitals BP: 130/85 mmhg, Respiratory rate: 25 bpm, Pulse: 75 bpm, Temp: 97.9 degree F. Examination On physical examination, she was conscious but severely cachectic. Investigations Chest x-ray shows haziness in the left lung and multiple nodules. Chest CT scan shows severe left pleural effusion with pulmonary metastases. Ultrasonograghy of abdomen shows a large mass (130*110 mm) in the left kidney. right kidney normal. Treatment Need your valuable opinion on the case.

Left renal malignancy with pulmonary metastasis and large pleural effusion Advise Tapping of pleural effusion It's likely to be renal call carcinoma with metastasis Previously renal cell carcinoma with metastasis had poor prognosis But now newer chemotherapy drugs such as Sunitinib has changed the outcome significantly with improved life span

* Left RENAL Carcinoma with . metastasis or secondaries in left lung. Needs proper assessment and clinical evaluation before treatment basically surgical intervention or chemotherapy.

The patient is suferring from diabetis melitus with Renal cell carcinoma with left sided pleural effusion with pulmonary metastasis . Advice pleural tapping, combined therapy

Adv PET scan. Evaluate for primary malignancy. Oncologist's opinion for further management.

A case of left renal cell Carcinoma with pulmonary metastasis which is one of the common sites of metastasis. Considering stage 4 disease targeted therapy has become now the treatment of choice Sunitinib 50 mg daily for 2 weeks followed by one week no medication and then 2 weeks same dose and one week rest . This treatment to be continued. But only drawback of this treatment is cost factor - Sunitinib - which is a multi -targeted receptor tyrosine kinase inhibitor is a costly drug .Treatment costs about 1 lakh per month

? Renal cell carcinoma with lung metastasis CT abdomen and biopsy from renal mass.. PET CT to assess spread of renal malignancy . Biopsy from lung mass and pleural aspirate analysis for HPE and immuno histo chemical analysis . Left nephrectomy followed by chemo radiotherapy.
