male Child age 1 yr with hard swelling since 1 day as seen in pic , No fever , child playful .kindly help in diagnosis and treatment. ear examination normal, throat examination normal. no signs of RTI

1 year child, Swelling Rt. side Neck for 1 day. No Fever. Rt. side Neck Swelling looks like: CERVICAL LYMPHAFENOPATHY. D/D : INFECTION IN THE DRAONING REGION SCALP. ORAL CAVITY, PHARYNGEAL INFECTION. RT. EAR INFECTION. R/O LYMPHOMA.

Oral cavity is perfectly healthy , pt still does not have fever , ear exam normal, but swelling has reduced after Amoxyclav. Usg neck and Cbc has been advised. Many thanks for your guidance.

Do imaging. USG neck. Routine Blood tests with ESR and Mountox. Till you evaluate give 10 days of antibiotics preferably amox. Seems to be cold abscess of tuberculosis. But have to rule out lumphoma also. i thought swelling is from 1 year. but its in 1 day. acute. so TB, malignancy, reactive lyphadenitis are major differentials. Consider taking history again.

Thank you doctor , swelling is reducing in size after Amoxyclav. CBC, ESR , Usg neck has been advised.

If it's really hard swelling and there s no fever n child is playful then first rule out malignancy then go for other infections.

Apparently looks like Cold abscess under tension. Other possibilities are lymphadenitis, lymphoma and organised haematoma

Submandibular adenitis and Ludwig angina ,

Submandibular lymphadenitis,do a usg for collection

It's cervical lymphadenitis Respond well with Amoxyclave &Ibupara

Submandibular or cervical lymphadenopathy Workup require as duration is one day it seems reactionary or inflammatory..
