Menorrhagia

A 19 years female presented with 4 months of amennorhea, and went for ogician opinion, tsh, coagulation studies and cbc were normal... Usg shows mild pcod... This month 5th july she got menses and she had heavy flow till today... Menorrhagia for almost 18 days... She was treated with tranexamic acid, iron and calcium tablets... But bleeding was not controlled..what is the next line of management??.

19 year old . Married PCOD. 5 months amenorrhea followed by bleeding pv since 18 days. First rule out pregnancy. Do Serum B HCG.if pregnancy test is positive, do quantitative serum B- HCG. DO ABO-RH. Repeat CBP. Do a transvaginal scan. See for endometrial thickness. Give IV tranexamic acid 2 ampoules in 100 ml NS. Give Tab CRINA NCR 15 MG daily for a month. This tablet should reduce bleeding. If bleeding is not controlled and endometrium is thick, go for D & C. . Please write history in detail like marital status , obstetric history, menstrual history etc

5months amennorohea Bleeding flow heavy Trans vaginal scan main Endometrial thickness PCOD Give inj.Tranexa with IV NS 100ml T.Overal L T.Crins NcR 10,mg daily give 30days If bleeding is not .reduced go for D&C Rule out B HCG main,repeat CBP

Could be talking oc pills. Also Get her coagulation profile.

She is not on oc pills... And coagulation profile is normal sir

Advised D & C Ref to gynaecologist

Not possible..rarer case to think of. Patient might have not provided proper history seems.. Plz upload the reports. Refer to gynaecologist..
