A 40 year old male presented with multiple grouped papulovesicular lesions on both elbows, knees, buttocks and upper back associated with severe itching. The patient also a significant H/O flatulence and excessive hair fall for the past 2 years. Additional h/o revealed weight loss of 10 kg and occasional episodes of Diarrhoea without mucous and blood O/E sunken eyes, scanty hair, unevenly spaced beard, and angular stomatitis. His Hb is 10g/dl, his serum iron, TIBC, and folate levels were all low. What is the likely dermatological disease? A. Bullous pemphigoid B. pemphigus vulgaris C. Dermatitis herpetiformis D. 28/F, a h/wife came with c/o prodromal symptoms of fever,malaise,loss of appetite,weakness x one week. Thereafter developed vesicles over the sole/feet ,palms and hands, followed by vesicles with ulcerations all over the trunk & abdomen and then in the mouth whereby she could not open and eat. Whole of palate, Cheeks and tongue has multiple mucosal ulcerations extending over the lips. No h/o of similar eruptions in the past, no familial h/o of such symptoms. Pts CBC is normal except for high Neutrophils and Hb of11.0gm%. HIV,RPR,HBs/HcAg nonreactive;ANA awaited. DD ?

40-year-old female, without comorbidities presenting with these itchy blisters all over the body for 3 months. Initially, it started as a single blister only. When they heal, they form dark crust. The mouth is also affected. Patient was prescribed flucloxacillin for 1 week without any improvement following which she was prescribed dalacin capsules with chlorhexidine wash for another week. No improvement as well. No fever. How do we tackle this case and what are the treatment options?
