Management of Nailbed injury depends on the depth of injury and local tissue loss and duration since injury. Injuries that present immediately with minimal tissue loss can be repaired after an Xray of the distal phalanx for fracture that needs stabilisation and repair of nail bed laceration with nail excision and evacuation of hematoma. Delayed presentations need only excision. Closed nailed hematoma presenting late needs only evacuation with a needle under local anaesthesia.
Check there is no fracture . If not then dressing with pain killers. Don’t remove nail. No need of any antibiotic. Nail is the best protection.

Patient complaints of blackish discoloration beneath the nail of thumb, only one half of the nail is involved. History of formation of absces in this area 1 year back which was drained. Yellowish brown accumulation seen beneath the nail. One half of the nail is detached from the nail bed, other half is firmly attached. She has applied henna to the fingers in this image.
