50F having c/o Rt Hypochondrial pain radiating to back for many months. 2 months before she went for USG abdomen which suggests multiple GB calculi.. Today she presented with extreme abdominal pain, vomiting and diarrhoea.. CBC suggestive of Mild leucocytosis. LFT, Amylase/lipase within normal range. Kindly give comments on following standing x ray & opine on further approach..!

Imran Hayder1 Like13 Answers 34 year old male presented with pain in right hypochondrium since 1 day, sudden in onset, rapidly progressing, colicky in nature, Non referred or radiating, a/w distension since 1 day and multiple episodes of vomiting since 1 day( non projectile,non billious, non blood stained), no fever/ normal bowel movt/no burning mict/no chest pain or dyspnea. K/C/O Abdominal koch's on ATT since 2 months. On examination:- tender abdomen. Bowel loops palpable. Viral markers negative. Liver enzymes:- ALT-15 ; AST-28; ALP-200 X-Ray(image attached) shows multiple air fluid levels,dilated bowel loops and no gas under diaphragm. Please discuss the next line of investigations and management.
