24yrs old married male complains of swelling and pain in right testis from 1 week after he met with bike accident, Doppler study shows EPIDIDYMO ORCHITIS RIGHT TESTIS WITH ENLARGED AND INCREASED VASCULARITY... treated with tab chymerol forte TID , Amoxclave 625 BD,tab prednisolone BD, But there is no recovery, pain will disappear when he take medications, it starts again after some time.. Wat are the further steps to b taken..??

p2l2, post lscs 14th day.. hard swelling near Rt side scar site Rt iliac fossa n around umblicus.. Pt asymptomatic since surgery.. Needle aspiration done.. it was collected blood.. but in usg report given as abscess.. v r suspecting hematoma.. how to manage

A 10 yrs old male child admitted with C/O-Lt Ankle swelling(XRAY-NAD) , Resp distress, vomiting, Fever, Abd distension with tenderness. H/O- Fall 7days back with Lt ankle sprain,Since then fever, vomiting started. Hr-160/min, BP-90/60 mm of hg, RS-B/l crepts,RR-70/min, P/Ab-distension with tenderness. Relevant lab invest done s/o neutrophilia, crp-positive. Symptomatic treatment was started. Inotropic supp was started. Fever was persistent, 2d echo done-wnl, CT A+P s/o-b/l pneumonitis and mild hepatomegaly, MRI lt ankle Jt s/o severe osteomyelitis with overlying cellulitis. U/O was adequate. By second day Pt developed vesicular eruptions all over body with swelling over Rt ankle joint and pain while passive movement. Needs ur further opinion regarding case.
