A female came to OPD for cosmetic disfigurement of b/l thumbs since 2 months Chief Complaints No pain or itching Progressive destruction of nail plate Diagnosis Approach ?

Dorsal pterygium. LP is most common cause. But rule out accordingly Rx Tacalcitol cream LAHS x 1 year or Intramatricial steroid injections

Onychomycosis Confirmation is required with KOH and Culture Treatment of Choice Terbinafine and itraconazole Duration might take a long time considering the severity.
