85yrs/F...c/o severe breathlessness since yesterday night 11 pm associated with frothy sputum... O/e pt is unconscious not responding to painful stimuli...pupils reacting to light, GCS 3/15.. Rs- B/l severe crepirations present along with b/l wheeze... Bp-190/110..Had history of wheezing 2 episodes one week before...but pt got stabilized with levolin nebulization.... Now pts breathlessness has not settles even after 3 cycles of levolin plus NS neb...pt was not taken to hospital...we visited her in her house... Gave 40mg lasix iv in view of acute pulmonary edema...inj hydrocortisone 100mg iv stat... Isdn 5mg sublingually... Pt's bp suddenly dropped to 98/60mm of hg...after a few minutes slowly increased to 140/80mm hg... Pt catheterized...only 300 ml of urine output was seen since 9 am in the morning till now... Oxygen was kept 4l/min...
