thalassemia case

A 28 yrs old female having complain of breathlessness since 3 years, initially patient was diagnosed as allergy to pollens since then she was on montelukast nd MDI , breathlessness relieved on medication nd aggravate on exertion, even on activities like laughing for 1-2 min(?severe dyspnea) , Patient exhaust on walking for short distance (100-300m), O/E -ECG , ECHO,XRAY ,CBC all are normal Patient having positive history of thalassemia minor What is the diagnosis or what investigation should be done further?

Obviously she is a c/o minor thalassemia and it's consequences her complaints corelate to it Get thalassemia profile done If pt is anaemic needs BT She should be on steroids in low maintenance doses Antihistamines+monteleukast Sos MDI if dysponic DOE suggest pt has cardiomyopathy and need to be evaluated in detail

PFT also done 6 months ago, that time it was normal Echo was normal

COPD with acute bronchitis.
