Left upper zone shows homogeneous radioopacity the lower border of opacity has concavity upwards It is suggestive of collapse of apical segment of left upper lobe There is obliteration of left costophrenic angle with air fluid level in left middle zone This may indicate left sided loculated hydropneumothorax or cavitatory lesion with lung abscess These finding point to pulmonary tuberculosis as differential diagnosis Adv HRCT chest Sputum for AFB gene Xpert and AFB culture

Dear Dr Jayesh, I don't agree with collapse in the lt upper zone because there's no ipsilateral tracheal shift ,hence I presume consolidation.
