Prolonge cough with newly diagnosed Hypothyroidism

Female patient of age 45 Chief Complaints Cough Chest heaviness On off sputum Loss of hair Loss of appetite History History of covid positive on 30/04/2021 snd turned negetive on 28/05/2021 No history of htn/diabetic/fever Vitals Bp:130/84 Pulse:90 Spo2: varying between 94% to 98% Physical Examination Pallor+ Anemic+ Investigations Hb:10.8 Esr:30 Tsh:10.8 Lft/kft/lipid profile: not significant finding Chest xray : Pulmonary edema?? With linear hazziness (10/07/2021) Diagnosis Need opinion Management Need opinion

A c/o hypothyroidism faced covid infection recovered of it and c/o consistent cough Hrct suggesting subpleural thickening of fibrotic aetiology suggest post covid lung fibrosis Rx antifibrotic drugs Tab pulmoclear 1bd Tab nintena 150mg 1bd Tab pirfenidone 200mg 2tds Sos oral steroids in low doses Duolin+budecort inhalation Stepup doses of L-thyroxine to 75ug od Tab dytor 10mg i1 od Keep followup

45/F..case of post covid 19 k/c/o:subclinical hypothyroidism. Report:mediastinal lymphadenopathy early fibrosis with ctss:9. Provisional diagnosis:Viral Pneumonia with fibrosis with Subclinical Hypothyroidism just repeat CRP-DDIMER-Sr Ferritin-IL6. Need to check previous hrct and line of treatment provided. any latest rt pcr done as hrct suggesting corad 6? Advise: Antibiotics with antifibrotics bronchodilator with systemic and in nebulization form. sos steroids to think of. supplemental LT4 drug to start. haematinics and multivitamins advised.

Anaemia Chest infection Hypothyroidism Needs with post COVID management. Other wise consult with an Endocrinology and Pulmonary department.
