ECG. Sinus Mechanism. Basic ECG is normal. Repeat ECG And take ||| lead in fully inspiration phase. q wave disappear. Take chest X-Ray. Do Widal or Thyphi Dot Teast . Give inj. CEFTRIAXONE
ECG shows sinus rhythm and LVH pattern TMT is adviced Chest xray
Sometimes Q waves in lead 3 associated with inverted t waves with no other complementary changes in other leads is normal variant which disappear when ECG taken in full inspiration.I agree with @Dr. Devraj Dholariya sir. Chest X ray needed.

Nasal oxygen, thrombolysis, vasodilator infusion, morphine if not contraindicated Bed X-ray chest, echocardiography and serial enzyme study. Angiography to guide surgery/stent implant
