scrotal swelling in newborn

A day 2 baby was brought by her APPREHENSIVE mother to OPD for scrotal fullness. Baby is otherwise stable , scrotum is non-tender. what is this ?? Both testis is normal on palpation and equal in size.

Looks like bilateral congenital hydrocoele/ hernia Adv Reducibility of swelling goes in favor of hernia and irreduciblity goes in favor of hydrocoele USG of scrotum for confirmation

It is definitely a case congenital hydrocele to differentiated from congenital hernia. Clinically to see the reducibility of the scrotal swelling if reducilbe it may be hernia if not reducible it may be hydrocele . If necessary Go for USG. Wheather hernia or hydrocele no active treatment upto to the age of one year.

Please do standard clinical test(getting over the swelling,fluctuation,trans illumination etc,if in doubt USG which is not required generally)This will differentiate hernia also. Bilateral Cong hernia or even bilateral hydrocele is comparatively rare.In a early neonate you may get fluid collection in scrotal sac due to process of delivery,persistent of processus scrotal and abdominal sac connection,which gradually disappear.This situation is common .So wait and watch!
