SEIZURES IN A CHILD - 10 YEARS OLD - ISCHAEMIC BIRTH SEQUELAE?

A 10 years old male child with history of recurrent seizures - since last 2 months. Remained admitted in hospital during seizure episodes twice . Now Child is stable on tab levetiracetam 250 mg × 8 hourly . A CE - MRI scan - reveals focal areas of encephalomalacia with gliosis seen in fight parietal lobe with ex - vacuo dilatation of right lateral ventricle and focal areas of gliosis in left parietal region. At present- child is stable , no neurological deficit . Further management in this case .

The usual direct cause of the encephalomalacia is a hypoxic event experienced by the infant, either in utero or shortly thereafter. Hypoxia is when oxygen is not getting to the brain There is no direct treatment or cure for encephalamalacia. However, doctors may attempt to treat the underlying cause of the condition, which cannot be reversed. In some cases, surgery may be performed to remove the part of the brain affected by the softening. The treatment is primarily supportive with correction of the underlying cause. The supportive care includes maintenance of adequate ventilation, metabolic status and vitals, and control of brain edema and seizure See for EEG Repeat mri after 2 to 3 month regularly And treatment accordingly
