Sudden sharp chest pain

Chief Complaint A 25 year old male complains of sudden sharp, chest pain. History He gives h/o productive cough for 1 week with mild chest pain, for which he took Demerol. He started smoking 4 months back with 6 cigarettes/day & drinks rarely. Vitals Vitals are wnl. Examination Physical examination is unremarkable. Investigations Blood report is normal. Ecg is normal. CT shows air involving the mediastinal structures Treatment What will you advise?

Pneumomediastinum Cough, asthma, bronchitis are more frequent causes of pneumomediastinum Treatment. The air in mediastinum is gradually absorbed from the mediastinum. If pneumomediastinum is accompanied by pneumothorax, a intercostal drainage tube may be placed. Breathing high concentrations of oxygen may allow the air in the mediastinum to be absorbed more quickly. Treatment of underlying cough is important. Cessation of smoking is very important as it is contributing to cough. Antibiotic may be prescribed to treat underlying infection.

We need to consider that Chest pain with cough that too smoker need to rule out bronchiectasis and also allergic brochitis Need hrct to rule out covid With covid RT PCR Now start with antibiotics and other antihistamine

To see extension of mediastinal air ,Please update CT thorax lung and mediastinal window.

By pneumothorax,if there is pneumomediastinum is accomplained ,then have to place intercostal drainage tube Cessation of smoking is important as it has contributing to cough. Start antibiotics to treat underlying infection.

Indicative of Pneumomediastinum./Allergic bronchitis. Needs drainage, cessation of smoking, antibiotics..
