Watercan perineum

Forty year old male presenting with complaints of pain,fever,swelling with discharge of pus extending from scrotal to perineal region with a history of one week.

It is purely a case of Fournier's gangrene with extension to perineum characterised by necrotising fasciatis of skin and underlying tissue. This condition may lead to septicaemia and may lead to shock so early treatment and intervention is required. IV drip for nutrition and to combat shock and electrolytes supplementation if needed . Antibiotic higher antibiotic and to shift according to culture and sensitivity report. Debribement of the of gangeneous tissues and dead tissues till the normal tissue is seen. and regular dressing . Many a time testes both the time get denuded may reqire to r transplant to medial side of thigh depending on the extent of gangreons changes of the skin and tussue if not total denuded regular dressing and if wound becomes healthy with welformed granulation then split skin graft may be planned.

A case progressing towards Fournier, s gangrene Treatment should be energetic 1.Inj .Piperacillin 4 gm +tazobactum 500 mg I/V - 6 hourly for 1 week 2.A little debridement is,required 3.IV fluids to prevent dehydration and electrolyte disturbances- if any

Forniers gangrene, needs treatment with broad spectrum antibiotics, incision and drainage and excision of all gangrenous tissue, with dressing till it heals, check for Diabetis and any immunocompromised conditions.
