Watery diarrhea in 14 year old male

14-month-old male presented with a 3 day history of watery diarrhea that has progressively worsened and is now green, mucusy, with lab confirmed blood (small amounts seen in the last pictures). Patient otherwise fine until day 3 when he developed fever 101.5 max. Diarrhea occurs about every 1-2 hours. No abdominal pain/nausea/vomiting. Decreased appetite but drinking fluids (water, mom still breastfeed, and also supplements with goat milk formula due to past suspected cows milk protein intolerance). No changes to diet except mom started to introduce formula milk and states baby has been eating eggs more regularly and used to have an egg intolerance which she thought he had outgrown. Stool cultures and allergy referral pending. Thoughts? Infectious or allergic?

It looks like a case of persistent diarrhea which initially started as a viral watery diarrhea with subsequent secondary infection and becoming dysentery with most likely shigella infection. It is compounded by some allergic component due to suspected cow's milk allergy. A sigmoidoscopy to look for characteristic lesions of cow 's milk allergy. Start with IV fluids, oral Zinc suspension, inj ceftriaxone 250 mg IV B.D , econorm sachet twice a day . Avoid Cow's milk and supplement with soya,milk and curd

It is a case of bacillary dysentery characterised by loose motions with abominal cramps if not treated properly it leads to loose motion with blood may be microscopic or macroscopic and frequent small qyantity with lower abdominal cramp. If the pt shows signs of dehydration then IV drip to continue Inj Ceftruaxone 250 mg iv / I'm bd Tinidazole suspension 1 tag 8hrly Blood for electrolytes and to take measure accordingly Symtomstically pt will improve but it will take 7 days to get cured.

Watery diarrhoea means mostly viral diarrhoea....but blood stained required usg abdomen....basic blood report like cbc,crp,na,k,creat,sgpt,esr.....start zinc syp, ors, iv fluids, sos antibiotic, stop topmilk....give rice,banana,pomengranate juice,apple, etc... Keep admitted till consistency improves

Its infectious diarrhea. Dysentery. Think of shigella salmonella campylobacter jejunum enterohemorrhagic e. Coli. Give inj ceftriaxone at a dose of 75 mg/kg/day and inj metrogyl. Give iv fluids. Paracetamol for fever. Cbc serum electrolytes stool r/e should be performed.

Bacterial Dysentery Inj ceftam 1gm iv bd Inj metrgyl 100ml TDS Inj buscopan 1amp I'm sos. Iv fluids and maintain electrolytes. Multivitamin and antioxidant. Supportive treatment. Improve General health and personal hygiene.

It is a bacillary dysentery, antibiotic with antiprotozoal,ors,Zink suspension,as the patient is 14months old
