What is the best treatment available for trigeminal neuralgias

85 yr female with trigeminal neuralgias since 15 days. Tested with Tegretol 600 mg OD for since last 10 days with some relief. What are the other treatment options available for the same?

Is she able to tolerate 600 mg single dose of tegretol?Carbamazipine / Oxcarb 1st line drug for trigeminal neuralgia. Start with small divided dosage and slowly increase. Only thing one should check hyponatremia. Other drugs are Gabapentin,Pregabalin,Nortriptyline.

Try the following: 1. Oxcarbamazepine BD 2. Gabapentin BD 3. Baclofen 10 mg BD Adjust the dose accordingly per relief

Add Gabapentine 300 mg bd. Percutaneous peripheral nerve block can be useful. Reassurance and councelling required.

Treatment is somehow difficult because of chronic case & aging factors Better option is Inj Botox, but it is costly Otherwise Tab Carbamazapine 100mg tds, increase the dose gradually 400mg bd Tab Gabapentine 100mg tds, increase the dose gradually 300mg bd Tab Beclofen SR 10mg odd, increase the dose 20mg od Tab Citicholin 500mg bd Continue medicine, may not be withdrawn

START CAP PREGABA M75AT BED TIME ALONG WITH TEGRETOL OINTMENT XYLOCAIN APPLY LOCALLY

Apart from Carbamazepine 200mg tid Pregabaline plus Nortryptiline can be added.
