Large gut obstruction is more likely than gastric outlet obstruction ,since there is no vomiting. Abdominal pain, distension and constipation are present. ,? Sigmoid volvulus , malignant obstruction Suggest CT with contrast, and colonoscopy

It is acute dilatation of stomach , it likely to cause electrolytes imbalance. To keep the stomach empty by rule's tube suction To correct electrolytes imbalance IV drip to continue to maintain nutrtrition Repeat electrolytes estimation.
